You can create a custom form in the backend of the website. 

These forms can be used for a variety of purposes e.g. a sign-up form for the public to register for an event or fundraising initiative.

When you are in the custom forms section of the backend you can click"Add Custom Form" green button in top right corner.


You can link the custom form to Campaign Monitor here if needed by providing the 'list id'. The data collected from the form will then be sent to campaign monitor. 


Once you have created and saved the Custom Form, you then need to embed it onto an RFDS webpage. 

To do so, go to the page you want the form to appear on and create a new content type called 'custom form' and click go. 

Then select your custom form from the dropdown. 

We reccomend you test the form to ensure it is working as expected.
